Acetabular Revision Total Hip Arthroplasty Using Morselized Impaction Allograft for Acetabular Bone Defect above Paprosky Type III

Abstract
Purpose: This study was undertaken to analyze clinical and radiological result for acetabular revision arthroplasty using impacted morselized allograft or auto bone graft for actetabular defect above paprosky type III.

Materials and Methods: This study involved 32 hips of 30 patients followed up minimal 2years(2~15 years 1 month). The acetabular bone defects were classified by the Paprosky classification. We inserted a cementless hemispherical cup by press-fit technique using impaction morselized allograft or structural auto bone graft at 3 case of paprosky type IIIB.

Results: The last follow-up average Harris hip score was 93. Only 1 cases were re-revised. A radiolucent line between bone and cup was seen in 5 cases, this line was less than 2 mm. And a radiolucent line between host bone and grafted bone wasn¡¯t seen any case. Vertical and horizontal migration of more than 4 mm were noted in 2 cases respectively, but reduction loss wasn¡¯t noted.

Conclusion: The clinical and radiographic evaluation of acetabular revision arthroplasty using impacted morselized allograft or auto bone graft for actetabular bone defect above paprosky type III showed excellent outcomes after a minimum follow-up of two years. We believe this method is a good option acetabular bone defect in revision total hip arthroplasty.
